Description

Getting to know your students the first weeks of school and building relationships with them is one of the most important things an effective teacher can do in the classroom.  You can accelerate this relationship building by surveying your students, their parents, and even their previous teacher at the start of the school year to gain vital insight into who each of your students are.  We created INPUT to Output, four carefully crafted questionnaires, built in Microsoft Forms to help you gain the Insight Needed to Prepare Unique Talent in your classroom.  Capturing stakeholder input from these forms provides decision-making data to promote positive relationships, to build classroom communities, and to implement effective instructional practices.  

While most teachers will figure out the information they learn from these surveys by the end of the first semester, having this insight from the beginning of the year helps build more meaningful relations faster.  There are a total of 50 questions that are open ended or multiple choice among all four forms and 88 total questions including the demographic questions.

INPUT to Output consists of four Microsoft Forms“Knowledge is Power” is a basic survey for students to get to know them.  “The Power of Truth” goes beyond the basic survey for students for you to learn more about them at a deeper level.  Encouraging students to share information about themselves can result in better performance.  “The Power of Three” is a short form to send home to parents to hear their perspective and request basic demographic and contact information.  Finally, “Power in Numbers” is a short form for their teacher from last year for yet another powerful insight into the student.  Obtaining input from families and former teachers helps to focus future communication and instruction, respectively.

  • “Power in Numbers (Former Teacher Survey)” Microsoft Form with 5 open-ended questions for the scholar’s former teacher to share information plus an added 4 demographic questions.
