This is the physical therapy version of my Beginning of the Year form. Attractive and functional it is a great way to advocate for physical therapy's role in the schools and begin a conversation with classroom teachers! A letter introduces the teacher to school-based occupational therapy, as well as the therapist's schedule and best ways to contact. Specific to the child; the form includes an easy way to record areas of needs being addressed in physical therapy, a time/day for scheduled sessions, and a section to record activities and exercises. Print these forms single sided, and staple into a colorful file folder for a quick and easy way for the teacher to keep important information nearby!
