This family questionnaire is a great way to jump start your school year. You will get a priceless look at your students through the eyes of their parents/gaurdians. You won't find a more complete questionnaire to help you learn about your students quickly. It includes basic information like what the student prefers to be called, birthday, and the best way to contact parents. It will also give you a glimpse at their previous school experience, their strengths and weaknesses, any noteable health information, and the parent's perspective of their child's personality.
The last page is for parents to ask their child about their different ideas and perspectives about school.
This is a great way to show families that you really care about their child and that your classroom is sensitive to different personalities and learning styles. It also shows parents that you realize that THEY are the experts on their child and you are teaming up with them to accomplish the same goal together - a great year in school.
There is a section for parents to write two goals that they have for their child this year. This is awesome for your first parent-teacher conference! I don't know about you, but my first conference of the year is usually a goal setting conference. You will already be one step ahead as you prepare for the conference as you know what the parent is hoping for this year and then you can build on what they say and add your goal for the child for the year.
I love reading through these as they are returned to school. I keep them handy if I need a phone number or email address quick at any time during the year. They are great to read through again a few months into the school year when you know your students more.
Although this questionnaire is extensive, it is not too much for parents to fill out. It is four easy pages and will just take them a matter of minutes and provide you with a wealth of information all in one easy spot!
