The first eight questions in this document will help students begin to reflect about themselves and their role in the classroom community. The second set of questions will help you, as their teacher, to understand their mindset and beliefs about each subject taught in school. The bottom part of this document will allow students to provide their input into classroom rules and incentives. This document can be used with regular education students grades K-8 and Special Education Students. This document can be reflected upon and analyzed by the classroom teacher. Using the results of this document, the teacher will be able to tally the results and identify how many/ which students have certain perceptions about themselves and school. I extend this worksheet by tallying how many students choose "yes", "no", and "maybe" and then display the results on the board for students to see reflect on our classroom beliefs. Halfway through the year I revisit the second half of this document "Mid Year Self Reflection and Check" to see how my students views and opinions have changed. The second half of this document, which is available on TeachersPayTeachers, also includes a section that asks for new input that better relates to the Mid Way point in the school year.
