This behavior chart is based off of a management system where grades are given for behavior. The top portion of the weekly behavior chart is for a student's overall behavior grade for each day of the week. The bottom portion of the behavior chart leaves room for the teacher to specify what area the student struggled with each day. That way, the parent knows specifically why their child may or may not have had a good day. This behavior chart reflects the same content that a report card would, except it is based on behavior and classroom choices, rather than test scores.
The specific areas of behavior that are represented on the behavior chart are: Practices Self Control, Considers Others, Follows School/Class Rules, Cooperates with Groups, Listens Attentively, Follows Directions, Works Independently, Plans and Organizes Work, and Completes Homework. Each day, you can mark an O for outstanding, an S for satisfactory, an N for needs improvement, or a U for unsatisfactory. I personally only mark the areas that stand out each day, and would have caused a child to have a consequence (positive or negative).
