This is a 1/2 sheet card used when it is necessary to help a student see patterns in classroom behavior that need to be monitored or communicated.
Behavior Goals are limited to 2.
"Positive Expectations" are correlated with "Negative Noticings"
Typically, I print 2-3 weeks worth of sheets for the student. It is their job to take the sheet to every class and have every teacher account for what they see in class. (This is a very short process for a teacher).
There is a small space for the student to jot some notes at the bottom. Ideally, this plan allows the student to take responsibility for their actions. Feedback is immediate and therefore more relevant. After 2-3 weeks, growth can be reviewed and discussed with the student.
Sheets can be turned into a teacher/councelor/admin or in some cases, directly to a parent at the end of each day.
This is a tool to help facilate communication and build in student responsibility and awareness for their choices and actions. In my experience, with this process I've seen results in about 2-3 weeks, but it can be adapted and repeated as necessary.
