This is a terrific behavior chart for students who may need some behavior support in the classroom. This is broken up into four time frames: Morning, After First Recess, After Lunch, and After Last Recess.

How I use this:
I ask the student after each period of time:
1. Did you stay on task?
2. Did you complete your work?
3. Did you keep your hands and feet to yourself?
4. Were you following directions?
Your questions may differ based on the purpose of your behavior chart. The student and I have a brief conversation about what their behavior looked like during that period of time and the student and I discuss together where the belong on the chart (Happy Face, Okay Face, or Sad Face) and why they belong there. I then have the student color in the appropriate face with a color of their choosing.
There is a section at the bottom for comments to note any excellent behavior or needed improvement for the day. For each smiley face the student earns a small prize, like a sticker. After an agreed number of stickers the student earns a larger prize like a pencil or an eraser.
It's always better to implement positive reinforcement and try to encourage a student than to defeat their spirit. This is especially true for special education students who need some extra support.
