This is a behavior chart that I use for students with ADHD and EBD to communicate with special education teachers and home. Have the student evaluate his or her behavior and/or feelings in each class using the emojis. Use the number scale at the bottom of the page to rate the behavior as well.
The student can carry the chart to each class so that, at the end of the day, we can evaluate how the day went. It is good self-regulation to be able to describe how the student's behavior was during the day and to analyze what could have gone better.
