3 Templates for individual behavior/ check-in charts. I have used these three charts for individuals with ADHD, Autism, and students with Emotional difficulties. You can taylor each chart to meet your student's needs. I used checkmarks for the first chart by the hour. On the second chart the student can circle basketballs for meeting targeted behavior within the allocated time frame. The student/s can self monitor or the teacher can monitor their behavior and behavior goals. They are meant to be positive. You can only earn a check or basketball. I also had students set their own goal. The third chart is great for students who may need to 'talk' or debrief about their day in order to stay on track. I staple the charts to a folder and show parents when we meet. It is a great way to show data collection. I ended up using the check chart with all of my students because they all like the idea. Only some students then had tangible/activity based rewards while the rest just had the satisfaction of completing the chart.
