I use this behavior sheet with my toughest kids. I give them each a goal for the week and I track when they meet the goal. For example, a goal might be "Only talk when the teacher is talking." I also set a number of chances the student gets each day. For example, if this student talks out of turn more than 5 times, they are not reaching their goal for the day. If the student reaches the goal, they get a sticker for the day. Students who get stickers 4/5 days of the week, get a reward. Students who reach their goal less than that get a punishment. They get to decide on the reward and punishment. This can be altered as appropriate.
