This behavior checklist set allows for parents to see both the good and sometimes not so good choices that their child makes. A behavior checklist doesn't always have to mean a bad thing....as good behavior should always be rewarded as well. For some students this checklist could be a useful tool to use on a daily basis to help them monitor their behavior....for others...it could be used periodically just to keep the parents in the loop as to what is happening.

2 different versions of a behavior checklist. Version 1 includes positive behaviors and interfering behaviors. Version 2 includes positive behaviors and more serious interfering behaviors.
A sheet to record the ABC's of behavior (the antecedent - what happened immediately prior to the interfering behavior, the behavior - what was the interfering behavior, and the consequence - what action was taken immediately following the behavior)
A behavior reflection sheet - the student is able to explain what happened, how they were feeling, draw a picture, and reflect upon a coping strategy to help the next time they feel that way.
A letter of apology template - allows the student to apologize for their actions and allows them to get a response from the victim.
