This checklist is to meant to be sent home to the parents of the students who misbehave in class. It's a good way to keep in communication with the parent about their child's behavior. It gives them a checklist of all of the behaviors that the student may have displayed in class that day, along with a section for comments. It should be signed with the teacher, but most importantly, it should be signed and returned by the parent as recognition that they have read and spoke to their child about the misbehaviors in class. This is also a good way for teachers to document these behaviors and keep them on record as data for further meetings with the parents or administration.
