This is a behavior clip chart that I use in my fourth grade classroom. It includes seven different levels: Exceptional; Role Model; Good Choices; Ready to Learn; Warning; Reflection Sheet; and Parent Contact.
Students begin each day on "Ready to Learn," and can "clip up" or "clip down" depending on their behavior choices for that day. You can determine which rewards and/or consequences should accompany each level on the chart.
This version is full color, and can be printed on white cardstock. I also have a black & white version that can be printed on colored cardstock, if you are interested!
To assemble the chart, just print, cut out the ovals, and mount each one on another sheet of sturdy cardstock. (I cut my background cardstock to 8.5" by 6".) I punched holes in each corner and tied the cards together with ribbon to make a long chart. I recommend laminating all the pieces for added durability!
