This is a little chart that I made to staple into students' planners each week. Anytime I have to "pull their clip" or "change their color," they have to write the code on their chart that corresponds with the offense. This lets the parent know every time their child misbehaves. Parents must initial each day, so we know they saw it. These charts can be collected at the end of each week or left stapled in to their planner each week. Both ways have been successful for my team.
