This is a form I used when I was the counselor at a PBIS school. We used it to help students understand their behavior and take control of it. We also used it for date tracking purposes....what time the most incidents were taking place, where, etc. That way we could implement different strategies to help (brain breaks, etc). This would be for younger students or students that might have difficulty reading a think time sheet. We also used this with buddy classrooms so that the student went somewhere else to fill it out and talk with a teacher that wasn't as frustrated with the student and calmer. Very useful!
