As a facilitator for the SIT / GEI Team, we are constantly needing an easy way for teachers to document behaviors for challenging students without the documentation requiring significant amounts of teacher time. This form, which can be easily personalized for your needs for each student, is a quick and easy way for teachers to accomplish this without having to devote hours of instructional time. My teachers love this! You can use one page per day per student and document all of the information you need SO easily!
This is a Word 2010 version. I have the Word 2003-2007 version available or a PDF version. If you do not have the correct version of Word, it will not display correctly.
All the teacher needs to do is make a tally mark beside the BEHAVIOR, the LOCATION, and the ENVIRONMENTAL FACTORS involved in the incident. This will save the trouble of writing down detailed information and will help keep all of the information straight. There is a place for notes and disciplinary actions, also. Trust me when I tell you that this is a time-saver! You will be so glad that you started using this document for behavioral tracking.
I use this with the teachers in my school to save them time when having to document behaviors that are on-going during the day without having to spend tons of time actually doing it. Using a checklist to track behavior, location, and environmental influences, it makes it simple to note all of this information in the blink of an eye rather than spend precious instructional time to do it.
