I used this as a detailed behavior documentation sheet for students when i taught middle school. It is a chart that can be ran off back and front and utilized for an entire nine weeks. There is a key to the infractions and lines for notes and info from conferences and phone calls. I had one per student as the need arose.
I used (and still do) a notebook daily to write details of all situations from heads down to talking or throwing paper. I would then transfer it to the sheet at the end of the day or during my planning. It made it easy when talking to parents because I had all the info there and if they needed specifics I could open the notebook to that date and find the entire scenario for them. I tailored this to my classroom and it is editable for you to put to change to your needs.
It worked very well when documenting for BIPs.
