This is a COMPLETE BEHAVIORAL INTERVENTION PLAN SET (12 pages) that you can use over and over for about the price of a burger and fries. What could be simpler and more convenient? 

This set of forms includes everything that you need to do a BEHAVIORAL ANALYSIS, FUNCTIONAL BEHAVIOR ASSESSMENT, QUESTIONNAIRES FOR TEACHER, PARENT, AND STUDENT, PARENT LETTER TO ACCOMPANY QUESTIONNAIRE, and a form for writing up a COMPREHENSIVE BEHAVIORAL INTERVENTION PLAN.


I have used these forms for 11 years, and I have found them to be an effective way to provide the best supports possible for students with a variety of needs. The process that I use follows:


Step 1: Complete questionnaires for teacher, student, and parent. This enables you to delve deeper into possible motivations and pay-offs that might otherwise remain hidden.

Step 2: Meet as a team with parents, teacher, administration, counselor, and other staff members who interact and have regular interaction with the student. If the student is older, s/he can also be involved in this meeting, however, that is a personal decision. During this meeting, a FUNCTIONAL BEHAVIOR ASSESSMENT is completed. This step helps the team to determine probable antecedents, triggers, pay-offs, etc. that are feeding negative behaviors. The team will develop a hypothesis that will serve as a basis to create the behavior intervention plan in the next step.

Step 3: After the FBA has been developed, the team can use that information to create an individual BEHAVIOR INTERVENTION PLAN. This plan is developed to determine how best to intervene, support, and plan for possible crisis situations. The plan will identify and list possible INSTRUCTIONAL, CURRICULUM, ENVIRONMENTAL, and LEARNER-BASED accommodations. Additionally, a means to collect baseline and on-going data will be established. An EMERGENCY PLAN and FAMILY ENGAGEMENT plan is also included.

This is a set of very useful forms that you can utilize over and over again to support your students and help them to learn coping skills, alternative ways to respond, and increase academic success.
