Create a student aware of their own behavior. Allow them to have ownership of their behavior following this log. When a student breaks a code of conduct they must sign the book showing the offense they made. For each offense follows a consequence that is aligned with severity of the offense. For a student that demonstrates positive behavior have the student sign the book to reflect their positive behavior which will be followed by a treat (modify to your needs). At the end of the week. I mark down all offenses and honors on an individual calendar (for purchase). When a parent calls in to see what their child did, I have evidence to explain to the parent in the student's own words.
In a world where conduct is questionable, documentation is everything. This log can help you in multiple situations such as a parent stating that you are picking on their child, document behavior trends, etc.
