This is an Excel file that works great for any middle or high school class. I fill out one template for each class and print it weekly; you'll need to input your student's names, the week (i.e. January 1-5) and edit the behavior codes to your preferences (it's set for my middle school ag. classes, i.e. points get removed for being on the phone or computer, talking, sleeping, disturbing others, etc. and each week day has a column. Edit for your class!).
I print out a new paper every Monday and it's how I keep track of student's behavior throughout the week. On Friday, when I enter the behavior grades for the week, it's very easy to take a quick look and enter them into my grade book. There's even a column for the grade. Keep these for documentation, it's great to show in a conference that Jimmy was on his phone on 1/4, refused to work on 1/6, etc.
This is also a great visual aid for my students when they see that they've lost their behavior points for the day. I have it set up that each day of the week is worth 20 points and the goal is to have no marks by your name for the week, which would equal a 100%.
