I used this tool for when teaching students with significant behavior concerns. I included three suggestions for how to use the document.
The document contains a behavior log broken into fifteen minute increments. This can be used for the teacher's data collection (for baseline data or for ongoing data collection to determine if an intervention is working) or this can be the intervention with the students seeing and understanding the document. If collecting data on a student, you can see when the behavior is most often occurring.
This is also a good document to share with parents (or even principals) so they can understand the extent in which the behavior is occurring.
