I had a particularly difficult class one of my years of teaching and out of necessity, created this little document. Some students were consuming all of my guided reading time with their undesirable behaviors. This was greatly detracting not only from the other students, but from the target student themselves seeing as the time was wasted on just getting them to stay on task. I used this after I had exhausted all forms of discipline and could not afford to keep wasting the other student's valuable learning time. They are little cards that you can print out and cut out. If a student displays behavior that warrants being removed from the guided reading group (weather it be with the teacher, or students only), they would receive this card to fill out/relfect then take home and get signed. If the card did not come back I held a detention in its' place. If it was a frequent occurrence, then there would be phone calls, conferences, etc. This was just one step to help gain control of a difficult class so that everyone could learn. It really helped and held the students accountable.
