Many of us teachers incorporate the Clip Chart behavior management system in our classrooms. This is a great tool to accompany this management system! It aligns with the "Parent Contact" level and is used simply to place responsibility back on the student for improper behavior. These handouts can also be adapted to your own behavior management system in your classroom.
Students reflect on their behavior that got them to parent contact and write a sentence explaining what happened.Then, they think of a way they could do better next time. Parents are to review the handout, discuss it with their child, then sign and return the form back to school.

+ Primary grades may choose to use the pictures or simple sentences.
I even brought back the behavior boxes to check due to customer feedback!
+ For older grades students have the choice for more in depth thought and reflection.
+ All forms require parent and student signatures so assure teachers parents have seen them.
I typically place these notes in their file, which keeps them handy for conferences or other meetings. For repeated offenders it becomes the perfect evidence of behavior for future behavior interventions.
