This behavior matrix is an awesome way to track a student's behaviors each week. Every week, a student gets one of these, and it travels with them to specials and back to homeroom in a class binder. If a student makes an infraction in the categories of safety, responsibility, or respect, he/she bubbles in that area and writes a quick explanation; however, if the teacher finds the student did something awesome, he/she may sign under that category. At the end of the week, the students self evaluate using the bottom portion, the parents peruse it over the weekend, and it gets sent back on Monday with a signature. This gives parents a heads up on the in-school behavior and even a glimpse at homework completion patterns. Feel free to tweak it as you see fit.
