I use this chart to implement a behavior modification plan. I come up with goals with the student and parents that I think are realistic and can be achieved. I then assign a point system for when the student is meeting his/her goals most of the time (ex. 4 points), some of the time (3 points), and when they still need improvement (2 points). Each day I add up the total and have the parents sign the chart. I usually staple the chart into the child's homework folder. I also add comments when the student had a great day or when I need to inform the parent of concerns in behavior. At the end of the week if the student has achieved the total amount of points needed to be rewarded (ex. 80 points) then they earn some type of reward.
