Our school uses the 'stoplight' system for managing behavior. Every child begins each day on green. If they break a classroom or school rule then they move their color to yellow. If the behavior persists then they move their color to red. I created this handy dandy note for parents that explains why their child had to move their color. The parent then signs the note and returns it to school. I like to save these for documentation of student behavior. If you want to get really fancy, print them on coordinating yellow or red paper so they stand out in the students' folders.
