I use this format for my behavior notebook. I use the first page as a cover for the notebook to give clear expectations. Our school is divided into 9 week grading periods. We have 4 different 9 weeks. At the beginning of each term, I go over the guidelines with my students to ensure they remember what I expect and the consequences. They sign at the top of their individual page that they understand. They also circle either 1, 2, 3, or 4 to indicate which set of 9 weeks the behavior log is for. Each time a student goes to sign the book, they must write the date the behavior occurred. I switch with another teacher and so if a student gets in trouble in her classroom, they circle her name. This allows parents to better understand what happened and from which teacher they could get further clarity on the situation. I send the sheet home with each report card and students start fresh. I reward students who don't sign the book each 9 weeks.
