This PDF File is a form that will guide you the process and make your data collection easier. As a special education teacher, I have helped many teachers through the process of collecting data on their student’s behaviors in order to determine the next steps for behavior interventions. This would be a perfect first step for RTI for a student with behavior problems.
This PDF File includes an interval Data collection form, directions, data summary pages, and questions that will help you begin to think about a behavior plan. The data summary form will show you at a glance how often the problem behaviors are occurring and also, where and when they are occurring most often.
Interval recording is best used for behaviors that last for longer periods of time. I would recommend this to be used for behaviors such as, staying out of seat for a length of time, off task behavior, refusal to comply with directions, or other disruptive or distracting behaviors that occur for a period of time, like excessive talking, making noises, or tapping on desks.
This product is included in my Behavior Observation Made easy Set. You get all three of my behavior observation data collection forms (normally $4 each) for $10.
