This was created for student's with severe disabilities who need visuals to understand their behavior but can be adapted for students in general education in the younger grades. I cut out each piece, laminated it, and put velcro on the pieces and pasted it on the student's desk. You place the "How am I doing today?" on top. Right underneath "How am I doing?", you place the empty box with a piece of velcro in the middle of the empty circle. You then place velcro on the other pieces. Depending on how the student is doing, you velcro the corresponding color on the empty square. If the student is doing well and on task, the student is on green and the green icon goes on top of the empty box. If the student is slightly off task but not demonstrating severely disruptive behaviors, then you give the student a warning by putting them on yellow. If the student is demonstrating severely disruptive behaviors (cursing, running around the room, hitting, etc), then the student goes on red. The consequence for being on red can be a call or note home, for example.
