Use this point sheet to track student behavioral information. This form is editable so you can track any 4 general behaviors (such as: stayed with the group, respectful, on-task, positive peer interactions) and 1 student specific goal per time-frame or class period (use the box on top to write out the goal).
Each time-frame/class period is worth 10 points across the row. Use the Behavior Points Key: did the student perform the behavior... 2=Most of the time ; 1=Needed significant support; 0= Needed constant, 1:1 support in each category per time-frame out of 10 points. The whole point sheet is worth 100 behavior points (10 rows).
I use this for a level system in my classroom. Level system: 100-85% green day, 84-70% yellow day, 69% and below red day. End of the day break choices are contingent with the student's earned color.
