This entire set was created to be centered around the Choice Reflection Forms. So here's a little more info about the forms:
*I use the forms to help students reflect on their "not so good" choices. We have a discussion about the choices made, and then their parents sign the bottom of the form and return it to school. The signed forms can be kept in a behavior binder or in student files. I have found these forms to serve as great communication between home and school. They also are useful documentation of students' behaviors.
*The Behavior Reflection Center bulletin board set can be used to introduce how the Choice Reflection Forms will be used in your classroom. This set can be posted in a designated area of your classroom for students to reflect on their choices, using the forms.
*The forms can be placed inside a Behavior Binder, where you can also place the Behavior Log. This log can be used to quickly document each completed Choice Reflection Form and when it was returned to school. This allows for comprehensive documentation for parent phone calls, conferences, and report cards.
