This behavior reflection sheet was created to have students to really stop and reflect on their behavior. A child must learn to recognize what contributed to their behavior, what their behavior was that got them in trouble to begin with, what consequences their action caused and finally what ways to better handle the situation next time. As an educator, we should be able to distinguish when would be an appropriate time for a student to fill this form out. I, personally, use this when a student has been sent out of their classroom to “cool off” in my classroom. I have a beanbag reading area in my room and they know to go there. Once they have calmed down I keep these forms printed out in a basket at this area and clipboards to use. This is like their “exit ticket” back to class. I try to find just a minute to conference with them about handling the situation better next time & back to class they go. I also use this with my own students when there has been an incident and I really want my students to reflect!
