When a student misbehaves or breaks a class rule, having them fill out a reflection sheet gives you documentation of the incident and allows the student a break to decompress and think about their actions.
There is space at the top of this sheet to either write or type a short list of rules for the student to circle.
The sheet asks the student:
What rule did I misuse? (circle)
What did I do?
Why did it happen?
What should I have done instead?
What am I going to do to earn back the teacher's trust?
At the bottom of the sheet has a space for your comment's regarding the incident. You may also add a signature line if applicable between you and the student's parent or guardian.
