Tired of assigning "lonely lunch", detention, and having students sit out at recess, only to have the same behavior day after day? Traditional behavior management not changing student behavior? Having students think about their choices and behavior is very useful in modifying behavior. This form asks students to think describe their behavior, think about what triggered it, and the consequences of that behavior, (natural and imposed). This form asks students to think of how their choices affect their classroom, peers, teachers, parents, etc... This form will be whole punched and will stay in student binder.
