PRODUCT DESCRIPTION

Classroom Management Forms

Behavior Clip Chart: I use a behavior clip chart (available in my store for free). Click “classroom management” under custom categories on the left side of my store. I reset the clips weekly on Monday. They do not reset daily.

The Book: I have a two inch binder called ”The Book.” I have printed page and slipped it into the clear cover.

Weekly Clip Chart: Inside ”The Book” I keep a about 20 copies of the weekly clip chart. On Monday, I write the date at the top of the form. Throughout the weeks, I keep a record of students as they move up or down. There is many options for quick checkmarks for offenses. I also have room to write in a quick note if the offense is not listed. I usually write the letter of the color the students is clipping up or down to in the box instead of a checkmark. Since pink and purple both begin with a “p,” I use a * for pink. I teach in a group with 3 other teachers; therefore, when an offense happens in my classroom, we all put the first letter of our last names instead of a checkmark under the classroom column.

Conduct Check: I made a few copies of page. I wrote my students’ classroom number and first name on the conduct check form. This set is the set I use as my original to make copies. I then, make copies and cut the copies. On Friday, I write the date and mark the form. I have a student staple the form in each student’s agenda.

Classroom Management: I keep several of these forms inside “The Book.” The first time a student gets on red, I call home. You may have to call several parents at first, but it really set a tone for your year. It also helps you build a relationship with your parents. If my students bring this form back signed the next day. I let them move back to green. The second time a student gets on red, parents are called in for a conference. The third time a student is on red, the student is sent to the office. (I have a list of behaviors that will result in clipping straight to red and or going to the office. When they are on blue, some student believe they have room to misbehave. In my classroom, it is clear that certain behaviors are unacceptable.

Witness Statement Form: This form is good if something happens involving several students. You may let them write down what they know and send it to the office if necessary.

Contact Sheet: This form is quick and easy it use. I keep all of mine in a large binder separated by class since I have multiple classes. I simply write the date and the reason for contact then circle the person contacted and the method.

Daily Travel Log: My students use this form to sign in and out to go the the restroom and other locations. This page travels from class to class with our students. It really helps us track those students that try to get out of every class.

Apple Record Sheet: This sheet has many uses. I write my students name next to their number and make copies. On Friday, I have a student call out the clips colors, so I can record the letter color next to each student’s name. My students earn a reward coupon (available in my store) if they are on green or higher on Friday. The reward coupons are tiered so the better the color the better the reward. As students use their reward coupons, I highlight their box.

Lunch Count Form: I made 36 copies of this form and I was done for the year. My students make their choice in the morning, and then the helper count and record how many students per choice.

While You Were Out: My partner teachers and I complete this form when students need make-up work.
