For each the skills a student demonstrates during an observable time period, give him/her a checkmark in the appropriate row. If the student does not demonstrate the skill expectation, please leave the checkmark area blank. You may leave any remarks in the comments area below.If a skill was not able to be observed during a given time period, place a dash (-) in the corresponding checkmark box. A percentage score will be tabulated based on number of checkmarks received out of a total possible.

Use this for tracking a studen'ts progress on an IEP goal or as part of the RTI/MTSS process. Retain multiple copies with the use of duplicate/triplicate/carbon papers.
