Copy the sheets back to back and cut in half. To make them more attractive, color the slips on bright paper.
I use a 3 strikes your out behavior system. You can use them however they work best in your classroom.
Here is how I use them in my classroom:
On the first warning, students take the sheet from the bin and place it on their desk as a visual reminder to stay on task and follow expectations. Students do not fill out the slips. If no other warnings are given, the slip is returned to the bin at then end of the school day.
On the second warning, students fill out the front of the slip by checking the appropriate boxes. The behavior slip remains on their desk as a visual reminder. If a third warning is not given, the student recycles the slip at the end of the day.
On the third warning. the student must write an explanation on the back of the slip to explain to their parents in more detail why they received multiple warnings. The slip then goes home for a parent signature and is returned to me the next day.
