These slips are life-savers when it comes to behavior documentation! I leave a stack of these on each desk in my classroom. Whenever a student is breaking a rule or behaving in a less-than-desirable way, I ask them to fill out a "pink slip." No immediate confrontation! The student fills out the slip and brings it to me at the end of class, when we discuss the behavior and possible consequence. At the end of the day, I take any pink slips and put them in a binder for documentation. If a student continues to misbehave, I have the slips to refer to when talking with the student/parents/administration! You can change them up and add your own rules and name, of course!
