This is a weekly star chart used to track behavior in a special education classroom, or for an intervention of a particular student. This was made in MS Word 2010 and allows for customization to fit the needs of your classroom.
In my classroom, a student earns a star every half hour if they are on task and following directions. If they needed some redirection they may receive a question mark in the block, or if they are out of control they would receive a red X. I use Blue Stars and Red X's. For those who receive all blue stars and no more than 3 question marks in one day they are able to choose a prize from my treasure box. 3 question marks in one day equals the same as a red X. If they get a red X they lose prize box, and they receive a consequence for each red X depending on student and behavior plans. At the end of each week, I make a copy of each chart file them in my student files and send home the original for parents to see.
This really provides a visual documentation for students, staff and parents to see how the student is doing behavior wise at school. It is good documentation for IAT and RTI information, it is also an easy way to see if their is a pattern to disclose to parents during a talk about possible medical needs for their child.
