If you have a student who is working on self-monitoring, self-awareness, and goal-tracking, this chart should work for your team! I used this with a high level of success for 6th and 7th graders in the school and home setting who are on the autistic spectrum (high functioning).
Included you will find an information sheet to guide use, a 2-page school chart (one section for each school day of the week) and a 2-page home chart (one section for 7 days of the week).
I have kept the format in Word to allow you to customize specific behaviors, the behaviors currently listed are kept in as an example. To maintain the formatting, please note that I used "narrow margins" as a setting.
