Behavior Student Communication Form (Stops Tattling)
This form was designed to stop tattling and class disruptions while making students feel that their student related problems are important and will be addressed. Students answer a set of 15 questions on their own time about their student related concern (usually at the beginning or end of the day, at recess, or at home). This form easily deters the student that doesn't feel their concern is important enough to take the time and effort to complete the form and offers the teacher the necessary paperwork when problems warrant assistance by the school counselor, principal, etc.
