This is a simple sheet where a student can track their weekly goals regarding behavior. This works well with Leader in Me, but is adjustable for any situation. A rubric is included to help students be responsible as well as show accountability with their parents.
Step 1: Student will set a weekly behavior goal.
Step 2: At the end of the week, they will write a brief reflection of how they did.
Step 3: Color in a box with green (met goal), yellow (did pretty well with goal), or red (did not reach goal).
Step 4: Students will write a brief thought for setting a goal for the next week.
Step 5: Students will show this tracker to their parents at predetermined times. My students will keep this in their homework folder so parents can see it daily if they choose.
This works wonderfully with upper grades and older students as they can focus on longer-term (week long) goals, versus younger students who do better with daily goals.
