This is a behavior tracking sheet for students to carry with them throughout the day. I used it for tracking progress on IEP behavior goals. Students would earn a + for class periods that went well. Students would earn a - for class periods that were not up to expectations. Students could also earn a -/+ or a +/- for class periods that had a marked change in behavior. A percentage was calculated of positives out of total scores given and entered into the IEP. This sheet also helped us to track how many breaks a student took in a given day. Sometimes we also asked teachers to track the type of "break." Such as a body break, a rest, a visit to the nurse, a restroom break, or a drink break.
SLANT and other behavior guidelines are provided on this sheet so supervisors can use the sheet for on-the-spot instruction of expectations.
