I wanted to create something for myself that would be very simple for my parents and students to understand, but, above all, simple for ME as their teacher to use for easy communication with parents. I place this behavior tracker in their Leader in Me folders, and it gets sent home for parent initials daily. At mid six weeks, I add comments and core subject grades so that parents can see where there child stands, and there is still plenty of time to pick up their grades.
This is how it works: at the end of the day, students will either color in a green, yellow, or red circle with their crayon. I have my own classroom management plan that dictates what happens if they get one, two, three red circles in a three week period, but that is up to you to determine. It's basically a kickback to kinder light system, but, hey, it works for me! Hope it does for you as well!
