This interactive behavior tracking sheet was developed specifically for use by special educators needing to track the percentage of time students are available for learning. This form helps to identify the percentage of time a student spends on-task, off-task, as well as document the time spent out of the classroom.
You can also easily track a specific off-task behavior to be reduced or extinguished. The convenient pull-down menu provides a list of behaviors to select. Pull down menu may be customized with the ability to write in any target behavior. In addition room is provides at each time interval to record antecedents, observable behaviors, circumstances and consquences.
