I use this document in my classes to track behavior and academic progress for the whole quarter. I give them out during homework check and I collect them at the end of the day. These do not leave my room.
Each day, students are given three points (three lines in each day's box). It is their job to keep these points. Behavioral re-direction costs them one point. The line is simply crossed out. Each point lost holds a consequence (1 = warning, 2 = reflection form, and 3 = parent contact). Remaining points are counted towards classroom or individual incentives. There is also a reflection area at the bottom of the sheet where they place something they are going to work on (next to the triangle) and something that they did well (next to the plus). I print this double sided to cover the whole quarter.
Grades are placed on this document daily in my class for homework and DFAs (Daily Formative Assessments). This is essentially a place to grade the "ticket out the door". These grades are averaged for the week and placed in the grade book. There is also a spot for a quiz or test grade for the week. To help my students gauge progress, I also write their overall average at the bottom of the week.
