I designed this "Behavior Card" for my students at my last school. These worked for HS and would work for younger grades, as well. They could easily be adapted for grade or subject preferences. I had the students pick them up as they entered. They would fill in their name, first thing. Each time they were doing something "off task" or breaking the rules of the class, I would take one card. It's nice because it is a silent cue to the student that they are not performing at the expected level of behavior. In my students' case, it also let them know that they were losing points. Then I would save them, and if needed, pull them out at Parent Teacher Conferences! this worked wonders for the students who claim to "not know" why they're losing points or eventually getting a referral.
