These assessment charts are simple to use and have proved highly effective.
The chart is in booklet form. Print two pages and then photocopy them back to back with the 'My Targets' section on the right hand side of the first page. I use strong coloured card and the children love colouring in the animals. Write the starting date above each grid. The finished product will have three grids on it so you can get a clear overview of the child's progress over three weeks.
The first type of chart is used by the teacher in school. I work out behavioural/ academic targets with students and monitor them three times daily. The grids contain room to place small stickers in the relative sections. These charts work best with the support of the parents. If the child does well they may wish to reward them too. I have included a colour and black and white version of the school chart.
The second type of chart is a blank chart which can be given to parents who want to work on specific targets for homework e.g. neatness or reading. I have left the 'Before/ After Break' sections blank so parents can write in subjects or specific target areas e.g. page ruled or completed within 40 minutes.
