In my classroom do bellwork daily as they enter class. They record their answers on this general template. At the end of 2 weeks, I total up the points and it counts as a quiz grade. This is double-sided, so it holds 4 weeks on one page.
It will really save you paper, copying, and time and hassle.
You can set up the questions on the projector, overhead or board, and kids know where to record their answers.
This template has been 'borrowed' by many teachers I have worked with, so I thought I'd share here too.
