When evaluating bilingual students, a number of informal assessment tools are KEY to doing an accurate evaluation. One of these fundamental elements is a parent language questionnaire to gather information about the student's language exposure and history. Every bilingual student is absolutely unique, and there are so many factors in understanding each student's needs. A comprehensive language history is important in parsing out language differences from language disorders!
This is a parent questionnaire in English. Translate while speaking with parents or with the assistance of a translator if you do not speak the native language of the student.
This questionnaire contains 3 sections:
1. Language Development: Basic section on overall language development, as you may have this information from previous reports or contact with the family.
2. Language Background: Information about student's language history, such as age of first exposure to each language, and home language information
3. Current Language Information: Student's current use of each language, areas of concern, intelligibility in each language, receptive language in each language
It is important to note that once you've gathered this information, it is up to you to analyze and reflect on your findings! This is simply an information gathering tool that is now at your disposal!
